Facilities/Event Request Form

Please use the Facilities/Event Request Form below for any meetings or gatherings that you would like to have in one of our spaces. We are blessed with a variety of options, but we also want to be sure we share space well, don’t overbook areas, and can provide you with the room set-up that you need for your group.

Requests can be completed and submitted in two ways:

  • Online: Fill out the form below and email it to our Facilities Manager, Marla Williamson, at mwilliamson@iohumc.com 
  • In-Person: Print and bring the completed form to the church office for Marla Williamson OR pick up a blank form from the church office and submit it in person. The church office is open Mondays-Thursdays from 8am-4pm. 

Once our staff confirms we can accommodate your request, we will let you know!
